A severe allergic reaction can prove fatal unless immediate and effective action is taken. Use our form to ensure you are prepared.

What is covered?

If you have staff (or students, residents, service-users etc.) who are at risk of anaphylaxis, it’s vital that to be prepared.

Using our document Allergy Emergency Action Plan will ensure your first aiders have the necessary information to hand if a member of staff suffers from anaphylaxis.

We’ve outlined the symptoms of anaphylaxis along with NHS recommendations on how to respond in the event of an attack.

The end of the form includes space for details of the individual’s closest relatives. Since these may change from time to time, it is advisable that the form is reviewed at least annually.

At the end of the document there are signature spaces, including one for the allergy sufferer to give their consent to the information being shared.
